I'd guess PMD as I can't imagine this happening to a coin in the mint process.
It looks like it was struck with a very heavy blow to me. The markings show scratches in the direction of the bald spots suggesting something was heavily dragged across the coin to cause the damage.
Anyway, that's my guess. Someone else with more experience may say otherwise...
Struck through grease... (then more grease added to the coin during circulation!)
