I been researching online for answers and have not found any pictures to compare to, until I found a thread here. It is 1995d quarter, weighs 5.12 grams, non magnetic it is about 32nd of a inch smaller (OD) I just would like to what is wrong with it.

Looks like it has spent some time in an acid bath. The acid will eat away at the copper core faster than the outer layers.
