1994-D-Quarter-Double-Mint-Mark?

RPMs stopped in 1990. That year they were punched was 1989. It is MD. Note the same affect on the '4' on the date.

Your coin has Machine Doubling (MD) also referred to as Strike Doubling. The devices are reduced in size, which is a marker for MD. Also, the doubling is shelflike.

Machine Doubling is common and happens due to a mechanical issue with how the coin is struck. Not a doubled die and doesn't increase the value of the coin.

Your coin also shows some Post Strike Damage as well, from taking hits in circulation.
