I've come across coins with areas that appear to be shaved off but don't seem to me to be PMD (admittedly subjective; little things like scraping that's limited to a finite area with no damage whatsoever extending beyond that area, the sort of thing that seems to me indicative of something that either happened during the minting process, or, the result of a precise machinist spending an inordinate amount of time carefully creating the damage -- for no reason).
Probably when the planchet is loaded in to the collar, hits the rim of the planchet and then the piece lands on the target area, and get struck onto the coin. After leaving the chamber, the rim bur falls off the coin.
