I'm not sure why the bottom portion of the right side of the W is missing.

Yes a so-called spike head and worth saving in a 2x2. With regard to the letters in the inscription, it looks to me like the die was starting to get into its later stages.

Probably because cracks can be a precursor of Cuds to come?
