Great question. I have no idea what caused the imperfection. Clearly there was something there (I am not sure a thread would produce this kind of an incuse mark). I guess since it's gold and the metal is softer, it wouldn't take much, and any obstruction would be readily reflected in the planchet after strike.
Here are better images pre slabbing. Sorry - capturing the details of a tiny coin in a slab using a phone is not a simple process.
