Judging from the pictures - this would involve two coins that are already struck, or one coin stuck to one die and one already struck entering the striking chamber again at the same time as a planchet. Seems very improbable to just happen randomly. Unfortunately it was withdrawn from the auction, it would have been an interesting one to see.

I wonder why it was withdrawn? possibly a fake? if real, it's a spectacular error.

Yeah, I wonder why too - and also would like to know if it had reeding or not. In any case, I don't think I've seen a two sided brockage before.

Those surfaces do not look legit - the first thing I would look at would be if there was any reeding on part of the edges, which might sway my opinion... If the reeding was absent, then it _could_ still be legit, (which could happen if the strike was broadstruck), but given the texture on the surfaces of the coins, I'd still be doubtful.
If it is legit, then most certainly shenanigans at the mint are responsible...
I think it would be very easy to recreate in shop with a good hydraulic press. All I would need is a genuine pure nickel 25c blank planchet and two normally struck coins....

Yes, I got this coin pulled..it was originally listed on ebay and the pictured revealed how off it really looks..if I recall, there was no reeding on it.

Good point Will. Makes sense that if someone manually pressed a planchet there would be no reeding on it .
