Thanks for doing this!! We get quite a few requests here for this form, since ICCS is still without their own web page... (and I don't see their business model changing).
I made this thread a sticky, so it resides at the top of the Canadian grading forum.
For what it is worth - I have built the same form in MS Excel.... so much easier for me to type (or cut and paste) stuff in... and also avoids any confusion with ICCS, since my handwriting is pretty bad.

Hey SPP-Ottawa, would you be willing to share your Excel template? This would be a nice time saver for bulk submissions :)

A member of Waterloo Coin Society has once again offered to organize a bulk submission and shortly after the announcement I was informed that the form on the club website was out of date.
Similarly I noticed this with other clubs and random search links and was quite pleased that that a forum member took the initiative to make the form public information. Have included the link in the May newsletter and saved a copy for printing if needed.
Had no idea the rates went up or that the 50 lot option was removed. Stopped going to the downtown Toronto shows where I would normally see Brian and briefly say hi.

Great news and frankly long overdue. Looking forward to some added features in the coming months.  I was getting worried that their days may be numbered but the Pandemic had finally shown them the need to become more accessible to all collectors, not just those attending meetings and shows.

An important thing to note, the webpage will time out after a certain time.
I just finished entering 115 coins... then "sorry, timed out" when I hit submit. grrrrrrr....
The good news is, you can save it as you go by hitting submit, then come back and edit using the "view" button. The nice thing is, it will give you a printable pdf.
Make sure you get everything in the right order, as you can delete a line, but not add one in the middle somewhere.
