Not an error. It is only an error if it happens during
the striking of the coin.
What you have is damage. PMD

@B58, first welcome to CCF. Second, I agree with @jimmyd that this coin was damaged--not a mint error. You can see remnants of the original letters.

Possibly caused by some sort of machine, like a coin rolling device.
