Hard not to miss the blackened ceter of this nickel. I`m wondering if a blow torch heat could cause both sides to bubble and give this effect.

I bought this seller's 2 legit strike throughs but I felt the other 3 were suspect including this one. blowtorch seems like a good possibility.
