I just see hits, circulation wear and flattening on the devices. No errors here.

The only two things I question whether it is or not is the start of the base of the 4 just below the horizontal bar, it is slightly wider than the rest of the vertical above the horizontal bar. The bottom horizontal on the 2 appears to be slightly longer than normal. Without clear closeup pics of the date it's tough to make a call. But with so much circulation wear it may be virtually impossible to attribute it. Interesting coin but like previously stated it is showing its age through circulation.

Here is a normal date:
1942-S-Washington-Quarter-Possibly-DDO-Or-RPM
With the circulation flattening the rim, I could see how the taper of the date, and the taper of the rim, could create what we see on this coin.

Yes understand if metal was flattened but that tail is raised

The only other thing that it could be is a die chip. It is not an RPM or OMM.
