Looking at the Obverse without magnification noticed something next to his eye. So looked further and it looks as if he had an extra eye off to the side. Right side of his eye. Than looked like his eye/eyelid was doubled as well. Looks Crazy to me.

It is definitely odd. I wonder if it isn't either the reverse impression of an eye from another nickel or else damage plus a little Pareidolia?
