As a guess I'd say it's PMD, but others may think otherwise... Sixpences were the most heavily circulated coin in Britain in the 1950s and 1960s and were used in lots of slot machines, so there was plenty of opportunity for them to get a bit battered...

Hard to say - could be a planchet flaw but maybe not.

Definitely has been damaged. PSD. The metal has been pushed in from the well placed hit it took.
