Someone familiar with German coins and medals should be able to help. In the meantime, if you can post a photo that is cropped to just show the medal, both front and back, this will help with identification. That dark patina certainly suggests it could be as old as the tag says.
I think the words in the border or legend would translate in Latin? I don't think it is a replica and I do find it very interesting. I can't read the lettering well. What I would do if I had it in hand, would be to jot down or type that text into google translate from Latin to English and then search the results. It may lead you to the story behind the figures in the medal.
