I believe it's one of two and whats going on behind his ear? And looks like some design imprint in front of ear

All the areas you are looking at, is showing reduction of the devices. So this is not a DDO / DDR, but just Machine Doubling. If it were a doubled die, then the devices would be larger than normal sized, rather than reduced.
