1980 P Quarter Filled In P And Doubling

1980 P quarter with the P filled in and there is doubling around part of the collar on obverse. The "198" is boogered up as well as the tops of all the letters of "liberty" what would cause this to happen or is it PMD

Looks like a minor MAD and LIBERTY 198 is damage from a coin counting/wrapping/vending machine. The mint mark might be a broken post and or circulation damage. A spender.
