The secondary nose and lips are an example of surface film doubling. It's caused by a thin film of lubricant on the die, the planchet, or both. A fairly common effect.
Error coin writer and researcher.

Nickels don't have reeded edges. It is quarter SIZED not struck on a quarter or on a quarter planchet. (quarter planchet wouldn't have reeded edges either.)
