I don't even know where to begin.
Both sides are flush, so I don't believe it was after minting punch or damage, but I'm the new guy and don't know.

Well it is damaged. Nothing in the striking process could alter a coin to look like that. Is the area in question magnetic?

Above the LIBERTY the rim has an appearance of being clamped which also shows on the reverse. the heart shape has a lot of missing material around it edge, and the appearance of tooling marks all around the inside heart shape tells me the coin was clamped and some tooling efforts removed the heart shape and the for unknown reasons was placed back into the coin ?
