The heavy thick rim suggests that it is a (clothes) Dryer Coin. Millions of small impacts inside the machine are responsible for what appears to be heavily worn surfaces.

Machine caused, but human error.
Post Mint damaged.

Clothes dryers are unhealthy places for coins.

Very common for a coin to come out of a pocket of a garment, and just bang around inside the machine.
