Probably a sharp pin was pressed through the plating. If you looked very close, that center will show gray. (Zinc under the plating) PSD

Under the loupe it appears that the plating has not been broken. It does look like that center ring could be an adhesive of some kind so maybe something was stuck on the coin protecting that area from environmental damage?
