The details on the "dios salve" side look hand-carved onto the coin; if it's supposed to be a mediaeval coin, then it may have been "tooled".
On the other side, I can't tell if it's been double-struck, or struck over another coin type, but there's clearly a second cross at 45 degrees to the main one.
