Has anybody seen anything like this before? It is perfectly aligned with the remainder of the designs on the coin. Curious whether it was pre or post-mint? Any insight would be appreciated. Thank you!

It is a counter stamped/punched coin, aka PMD. Not an error.

You can buy sets of punches on ebay and mark anything this way quite easily.
