Don't know if this happens often. Usually I see the die breaks and leaves a blob of metal filling the letter. Here just a chip broke from the die leaving the perimeter intact and just a dot inside the O.

That is unusual. It doesn't look like a "traditional die chip". I am not suggesting anything, as I can't be sure what caused it but it is nicely rounded and flows smoothly at the edges of the "dot" into the open portion of the "O".

Looks like a die dot. You find them in the fields and is like a pot hole starting to form on the die leaving a raised area on the field. They usually continue to get worse with age. But probably one of the real round ones in an unusual location. I would have probably missed that one, but it shows you are looking very closely.
