The metal got pushed over and flattened, from a well placed hit. Not an error and just damage. PSD (Post Strike Damage). Just a spender unfortunately.

constant use in vending machines will alter the devices on the outside edges of coins. Sometimes even coin wrappers that crimp the end of the tube will damage coins. A clue that sometime mechanical altered the coin is the affected area will be parallel to the rim:
