Looks like this is rotated approximately 135 degree.
I guess this is a very late die state.

Has this error been listed anywhere?

Another nice find @lom. Do your think that the roughness (especially on the rev) is from this being a late stage die or is just environmental/chemical exposure?

I am not sure... but the weight is 4.96grams.

In this box of nickels, there are a few in different years that has this same type of look.

@levelsofmadnes, If all of the coins with that look are dated 2005 and earlier, they could be Katrina coins. Many coins submerged in the toxic soup after Katrina were acid-washed. Katrina copper cents have a tell-tale orange look. Katrina CNC coins have the oddly orange copper layer and lightly pitted clad layers. I've seen some pitted Katrina five cent pieces, but they haven't been quite as pitted as yours. Just a possibility.
