Not a production error, me thinks. Looks like deliberate damage inflicted by someone. Photo of obverse might tell more of the story.

Note the rims where is was, likely, clamped during machining.

I would be willing to bet this was part of some type of jewelry or clothing. That looks like a blob of solder to me. It was machined a bit to give a recess, solder dropped in and attached. Thats my thought.
