Found this coin. Not sure if it could be improper alloy mix, poor annealing, or just severe damage? No idea what's going on on the reverse - multi color splotches and what appears to be another rotated wheat stem going on to the rim of the coin.

Is the overlay in the last image just a funny coincidence PMD? The way it lays over the wheat stem and goes on to flip up on the rim?

Yes exactly--just a coincidence. Note that this linear mark is incuse, but if this coin were double-struck, then it would be proud.
