1969 S LMC Initials G Appears To Be Struck Of The F

What causes this? The G appears to be double/triple struck and cutting most of the F in the initials. The initials are not a separate die, a die for the F and one for the G, correct? Machine Doubling? Also, best guess on a grade? AU-5?
