"CIVITATIS WIENNENSIS" means "of the city of Vienna" so this appears to be a medal commemorating something that happened in Vienna in 1284 (establishment of the Vienna city council?). The two headed eagle is a symbol of the Hapsburg dynasty, which began close to this date (Wikipedia says 1278), and the white cross on a red shield is the coat of arms of the city, which dates to about the same time.
