All I can say is it has something wrong, flattened, but not sure what would have caused it.

Here is another 2000 D Virginia Washington quarter and looking at this quarter and the one above I hope you can see the reason I questioned it.

Incuse marks on a coin are 99% of the rim, coin damage. Only 1% of the time are they a mint error.
