PMD can happen in so very many different ways it likely impossible to know what happens to everything we find. It also looks like the cent was in circulation awhile after the damage was done, so sharp edges were worn off making the damage look more weird.

I'm guessing that it was used to "penny" an old style fuse in a fuse box. If money was tight or they didn't have a new fuse to put in they'd slip a penny in where the fuse screws and tighten the blown fuse on top of it. Thereby bypassing the fuse and creating a dangerous situation.
