Who knows, but it did not happen at the mint.

There are a billion ways a coin can be damaged.

There is a finite number of ways an error coin can happen at the mint.

The question should be "How can this happen during the minting process?" If it can't, then it's just damage.

@gui, I agree with @bobby that this coin is badly damaged. Sometimes we attribute bubbling of clad coins to excess heat--so-called campfire coins.
