Cannot Tell If This An Error Or What Kind It Would Be

I found this note in my change and it looked odd, with the marks on the front and back of the note. I looked on Google to see if I could find a similar note, but nothing quite like it. Can someone tell me what might have happened here? It looks like it might have an imprint of some sort. Thanks in advance, A Report this Post to the Staff
