Just wondering how a planchet ends up this way and how often the roll out this way.

Looks like extreme plate bubbling.
No extra value but keep as a novelty if you want.

Can't pin point it, but it is a plating issue.

Not a planchet problem - I agree that the problem happened when it was plated. The reason the plating ended up looking like this, I have no idea.
