I'm New at this coin collection thing. Can someone help me out with this. I want to know if this 2000 D nickel left this way from the mint.

If you see dimples on the edge of the coin:

It was squeezed. The coin will also be out of round.

If it is still round, then the coin was altered. Either way, it is a damaged coin. A spendable one. (probably won't work in a vending machine)
