Question. Is the area in question, raised or flat. Raised would mean, it is a die issue. Flat would mean it was damaged by the strike. So is it raised or flat?

Well in the first run of images, the last one is that set is showing a flattened area with MD on that area:
1983-D-Quarter-With-Double-Ear-I-Think
Machine Doubling makes it look like it is raised, but isn't on this one, the MD pushed aside the metal.
These are Machine Doubling. On a doubled ear, the affected area is raised like an island:
