Question On Jefferson Nickel Error?

To start out I don't have a picture, It was shown to me quickly.

It is definitely identified as a Jefferson nickel by the Monticello on the reverse. The reverse, Is in mint uncirculated condition, But the obverse is almost Unrecognizable? You can make out a faint outline of Jefferson, But the date and any of the Lettering is indistinguishable.. Almost has a kind of Melted look to it. I have never seen anything like it, And I was hoping somebody else has and can shed some light on it for myself as well as passing the information on to my friend..

It could be an error or it could be damage.
Without pictures we would only be guessing which it is.

If it's an error, I would guess that a heavy grease strike-through is the most likely, but it could be other things like struck through a die cap. Standing by for pics...
