Probably done by the same guy who was caught creating these mint errors, hoping to find them outside of the mint.

Mr. Hound, I won't speak for Coop, but what are the odds this was an accident?

Think about everything that would need to happen for this to occur as a natural error and there be only one and also it goes unwaffled and makes it out into the wild. I, personally, believe this is what we call a mint assisted error.

Don't get me wrong, I'd love to own this, with it without its value, but I don't see it as a naturally occurring error.
