1964 D Cent. Multiple Errors Or Just Damage.

The face is actually pushed out off of coin I'm guessing from rear mark. It is thinner running down the middle top to bottom than right or left side. Thanks for any help. Wouldn't it be illegible if it were that much wear. Top of head and bottom of bust. Tapers on both sides but still read everything clear we and cent are clear as is states and bottom of bust. With this level of circulation wear plus those gouges, I think it is likely that the coin has been partially flattened. An alternative would be that this was minted on a partly thinned planchet, but that seems very unlikely. Can you please post the weight of this cent? Thx.
