1970 D Dime.transitional, Errors, PMD?

I have a 1970 D dime with a lot going on. Not sure if all, some or none are mint errors or PMD. I will post picture after each question so I dont confuse myself or anyone else. I know its a lot but thank you for looking.

1) Am I right to assume this is a transitional error coin...1970 D dime with the 1968 S proof reverse

2) As you probably already noticed the big "mark" on Roosevelts face. I used a mirror to take a picture(which I am not very good at) but as you can see the torch lines up perfectly with the mark on his face. Can someone please tell me the name of this error?

3) Almost all letters on obverse and reverse are damaged in one way or another. Not sure why or what caused this

Nothing is doubled. Looks more list distortion from partial areas affected by grease.
The reverse is listed as 1970-D 10C Rev of 68 FS-901
