My uncle gave me some coins and this was included. What could have caused this? Could the planchet have been damaged before striking? Note: the lines are on opposite sides, i.e., they are on the right side of the obverse and the left side of the reverse. Also, the lines are not parallel from front to back.
Thank you!

Seems to be a vise-job of some sort. Definitely not a mint product.

This one has some damage, but that is still pretty cool of your uncle!
