Nice example of a counter-punched coin. Some people collect these.

You can make them yourself with a punch set and a hammer.
Considered damage although people do collect certain ones.

JimmyD has this right, it was done with a hammer and a punch. Search on "counterstamps" here on CCF (search box is in the upper left of every page here on CCF) to find a whole collection of them.
