There looks to be an incuse area adjacent the 4's appendage, and that usually means metal was moved from a hit. In general, any coin in that sad a shape will have unusual looking elements.
It is impossible during the minting process for part of another number to suddenly appear under another one. What you have is some sort of PMD, not an error.
