This is not an error that occurred at the mint. It is another coin that was smashed up against this one making that impression.

Agree, appears to be post strike damage--possibly intentional, possibly not. May we see a photo of the reverse?

That is known as a vise job where another coin is squeezed against that coin.
Not an error but damage.
