Keep in mind, that surface scratches don't always affect the incuse device. When it does that deep, then it can affect the device. But on this coin is is just a surface scratch. This coin is showing a contact mark so deep that the bottom of the incuse device was affected. Often rotating the coin 90 degrees will show this better:
