It's a DDD ( Die Deterioration Doubling) for me. Experts might disagree, but I've had a coin with doubling like this and I posted it on this website and everyone that commented convinced me that it was indeed a DDD.
I think it is coin wrapper machine damage. The machine puts a circular scratch through the end coins, slicing any of the devices in its way.
I don't think it is DDD, this is too dramatic and square.
