I've never seen a coin with this type of damage.
What could have caused the square holes?

@1coi, yes good job correctly identifying this as damage. These square indents can be caused by the diamond-patterned jaws of a vise.

It may be damage from the jaws of a vise or something like that. No idea what someone was trying to do, but it's almost certain that this was intentional.

can't tell you exactly what caused it, maybe someone bored and went at it with a screw driver, no matter the cause though this can't happen during the minting process which makes it damage.

Just my observation here, but the coin has a slight bend. I wonder if the person that damaged the coin was trying to flatten it back out. I say this because the deepest of the square indentions are on the rim where the coin is bent, so it looks like the vise was used after the coin was bent.

I know when I was a kid, I would have tried to flatten a bent coin "just because!"

Only my opinion.
