Appears to be some kind of souvenir medal for the city of Nara, Japan. The observe reads "Nara". On the reverse is a poem composed by the poet Abe no Nakamaro in the 8th Century. You can read it here under poem number 406 in the classical Japanese poem collection, Kokinshu, :

*Nakamaro came from Nara and Mount Mikasa is located near the city.

I'd add that this looks like a souvenir key fob. Nara has a famous park inhabited by deer.
