Found this coin in change & I'm not sure this happened at the mint. Thank you for all the help I've learned am incredible account so far. Truly thanks for everyone's patience with me.

intentional damage on this one. It looks like some kind of tool was hammered into the coin. Notice the corresponding bumps on the reverse.

It is only an error if it happens during the striking of the coin.
As this was impossible to happen at that time, it is PMD.
