If it's not RPM-002 then I think it should be another RPM variety. In my opinion the first MM punch was weak so the MM device showed slim, then the repunch MM was a broken die so the middle section of the MM was being thick and wide.

Looks more like a glancing contact mark. Note the two extending edges on the hit area.

Note on the RPM's the second punch is visible. This one look like a single punch with a contact mark, moving the metal around a bit.
